I've never seen this before.
I'd love to hear details of the environment it's running in.
OS, Asterisk version, etc.

As the level of INFO indicates, this is not an actual error condition.
VXML interpreter threads routinely indicate their termination by simply
disconnecting from the managing process.  The code of -3 simply
indicates that the filehandle became non-selectable.

If there was no error logged, we do indeed have a mystery.
If it happens again, I'd love to see an strace of the process.
